Please call us at 662-323-9726 to get your child scheduled and to provide relevant information including a valid email address for you. Once you receive an email from us, please click on the special link directing you to our patient portal. From there follow the directions to complete your new patient paperwork online.

Medical/ Dental Office New Patient Referral Form

If you are a staff member from a medical or dental office and you would like to refer one of your patients to us, please download and complete our Referral Form. You'll need to save your entries and then email the completed form along with radiographs to the email address shown on the form. Thank you!
